Job overview

This is an exciting opportunity for a caring, compassionate and motivated  individual to work with our Clinical Nurse Specialist team as a Cancer Care Navigator working specifically with the colorectal patient group.

The successful candidate will work alongside our existing Navigators and is available for a fixed term of 2 years, the successful applicant will be an integral part of improving colorectal patients pathways.  

Main duties of the job

The purpose of this role is to complement the existing Colorectal team.  To help the patients through diagnosis, treatment pathways and follow up. To ensure the provision of safe, seamless, appropriate service. This helps maximise the health and quality of life of patient and carers.  The role will involve a significant amount of patient contact on a regular basis.

Working as part of the Colorectal team, offering and following up Holistic Needs Assessment, coordinate the care of the person living with cancer non-complex needs so they can access the right help at the right time.
